Coordinated Support for Independent Living

Home and Community Based Services (HCBS) Case Management helps older adults and individuals with physical disabilities access the services they need to live safely and independently in their communities — rather than in a nursing facility or institutional setting.

The HCBS program provides case management assistance in coordinating support and services for adults age 65 and older and for adults and children with physical disabilities. The goal is to help individuals remain in their homes and communities with the services they need, rather than moving to a nursing facility or other institutional care setting.

Your HCBS case manager serves as your primary point of contact for navigating the service system. They assess your needs, develop an individualized service plan, and then coordinate the delivery of services such as adult day health care, personal care assistance, respite care for family caregivers, home modifications, and other community-based supports.

Case managers also monitor the services you receive to ensure they continue to meet your needs. As your situation changes, your case manager can adjust your service plan and connect you with additional resources. This ongoing coordination helps prevent gaps in care and reduces the likelihood of hospitalization or institutional placement.

HCBS Case Management is a Medicaid-funded program. To qualify, individuals must meet Medicaid financial eligibility requirements and demonstrate a level of need that would otherwise require institutional care. Our team can help you understand the eligibility process and assist with applications.

What to Expect

The process begins with an assessment of your needs, current living situation, and goals. Your case manager will work with you and your family to develop a service plan that identifies the supports you need and the providers who will deliver them.

Your case manager will stay in regular contact to monitor services, address any concerns, and update your plan as your needs change. There is no cost to you for case management services through this program.
