Your First Step: What to Expect with IDD Services

Navigating services for yourself or a loved one with an intellectual or developmental disability can feel overwhelming. We’re here to make it as straightforward and supportive as possible. Here’s what you can expect when you reach out to RiverValley for IDD case management services.

Getting started is simple. Call our Point of Entry team at (270) 689-6879 and let them know you’re interested in IDD case management services. They’ll ask a few questions about your situation and connect you with the right team member.

Step 1: Initial Conversation. A member of our IDD team will speak with you (or your family member or guardian) to understand your needs, current situation, and what you’re looking for. This is an informal conversation — there’s no pressure, and no preparation is needed beyond being ready to share a bit about your circumstances.

Step 2: Eligibility and Documentation. Depending on the program, you may need documentation to verify disability, such as a psychological evaluation, individualized education plan (IEP), or physician’s records. For Medicaid waiver programs like the Michelle P Waiver or SCL Waiver, you’ll also need to meet Medicaid financial eligibility requirements. Our team will walk you through exactly what’s needed and help you gather the necessary documentation.

Step 3: Assessment and Service Planning. Once eligibility is confirmed, your case manager will conduct a thorough assessment of your needs, strengths, preferences, and goals. Together, you’ll develop an individualized service plan that identifies the specific supports and services that will help you or your loved one live as independently and fully as possible.

Step 4: Service Coordination. Your case manager becomes your primary point of contact for navigating the service system. They coordinate with providers, monitor the delivery of services, advocate on your behalf, and adjust your plan as your needs change over time. You’ll have regular check-ins to make sure everything is working as it should.

For emergency situations: If you or a loved one with an IDD is in an urgent situation — such as a sudden change in living arrangements, a caregiver crisis, or behavioral emergency — our State General Funds (SGF) program can provide short-term case management quickly, even while longer-term waiver applications are in process.
