Emergency and Short-Term Case Management

For adults with an intellectual or developmental disability who need immediate support, RiverValley offers State General Funds (SGF) case management services on an emergency or short-term basis. SGF case managers help clients access the medical, social, and educational services they need to live independently and participate in their communities.

State General Funds (SGF) case management is designed to fill a critical gap for adults with intellectual or developmental disabilities who need immediate support but may not yet be enrolled in a Medicaid waiver program. Whether someone is in an emergent situation, waiting for waiver allocation, or simply needs short-term coordination of services, our SGF case managers are here to help.

SGF case managers assist clients in receiving medical, social, educational, and community-based services. This can include helping with healthcare appointments, connecting individuals with housing and employment resources, coordinating with other providers, and advocating for the client’s needs across multiple systems. The goal is to stabilize the individual’s situation and ensure they have access to the supports needed for safe, independent community living.

This program is particularly valuable for individuals who are experiencing a crisis or transition — for example, someone whose living situation has suddenly changed, a caregiver who can no longer provide support, or an individual who is being discharged from a hospital or institutional setting and needs help coordinating community-based services.

SGF case management is funded by the Commonwealth of Kentucky and is available to eligible adults in RiverValley’s seven-county service area. Because it operates on an emergency or short-term basis, it can often be accessed more quickly than waiver-based programs. Our team can also help identify longer-term options, including the Michelle P Waiver or Supports for Community Living Waiver, and assist with those applications when appropriate.

What to Expect

Because SGF case management is designed for emergency and short-term needs, the process moves quickly. After an initial assessment, your case manager will begin coordinating services right away — connecting you with medical providers, community resources, and any other supports needed to stabilize your situation.

If longer-term case management is needed, your case manager will work with you to explore waiver options and help transition you to the most appropriate ongoing program.
