Intensive Team-Based Support for Serious Mental Illness

Assertive Community Treatment is an intensive, evidence-based program for adults with serious mental illness. Our multidisciplinary ACT team delivers comprehensive wraparound support wherever you are in your daily life, with crisis support available 24 hours a day, seven days a week.

The ACT model brings together a team of mental health professionals—including a psychiatrist, nurses, therapists, case managers, peer support specialists, and vocational specialists—who collaborate to provide coordinated care tailored to your needs.

Unlike traditional services, where you visit providers at scheduled office appointments, ACT team members come to you. Services are delivered in your home, workplace, or neighborhood, helping you build skills and stability in the real world.

ACT is designed for individuals whose needs have not been met by traditional outpatient services. This may include people who have experienced frequent hospitalizations, difficulty engaging with office-based treatment, homelessness, or involvement with the criminal justice system.

Our team takes a whole-person approach, helping with psychiatric treatment, housing, employment, physical health, substance use, benefits, and family relationships. Services are flexible—during difficult times, you may see team members multiple times per week, with contact naturally decreasing as you gain stability.

What to Expect

Services begin with a comprehensive assessment conducted by multiple team members to understand your history, strengths, and goals. You’ll be assigned a primary contact, but you will get to know all team members over time.

The team meets regularly to coordinate your care. ACT is a longer-term service providing ongoing support, with the goal of helping you achieve the greatest possible independence. Some individuals eventually transition to less intensive services.

Therapeutic Community Support

TCSS brings mental health support directly to your home, workplace, or community. Our professionals help you build coping skills, manage stress, and overcome real-world challenges in the environments where they actually happen.

Case Management

Your case manager serves as your advocate and guide, helping you access healthcare, housing, employment services, and other community resources while coordinating care across all your providers.

Peer Support

Connect with trained professionals who have their own lived experience with mental health recovery. Peer specialists walk alongside you, offering genuine understanding, practical guidance, and proof that recovery is possible.

Therapeutic Rehabilitation

Our structured day program helps adults with serious mental illness build daily living skills, social connections, and confidence. TRP combines skill-building groups, therapeutic activities, and a supportive community setting.

Recovery Care Coordination

For those who have achieved some stability, Recovery Care Coordination provides consistent support to maintain your progress. Your coordinator helps you stay connected to resources and navigate life transitions.

Outpatient Mental Health

Receive professional therapy and counseling while maintaining your daily life. We offer individual, group, and family therapy using evidence-based approaches like CBT and DBT, with telehealth options available.

Outpatient Physician’s Clinic

Our outpatient psychiatric clinic provides comprehensive evaluations and medication management delivered by experienced psychiatrists and psychiatric nurse practitioners for a wide range of mental health conditions.

Intensive In Home Therapy

IIH brings comprehensive therapy directly to families in their home. Designed for children and adolescents who need more than traditional outpatient care, our therapists strengthen the entire family system with multiple sessions per week.
