Your case manager helps you understand your options and connect with services across the full range of needs that affect your quality of life—including healthcare, housing, food assistance, transportation, employment services, and disability benefits.

One of the most important roles of a case manager is care coordination. If you receive services from multiple providers or agencies, your case manager ensures that everyone communicates and works together to prevent gaps in care.

Your case manager gets to know you as a person, not just a diagnosis. Together, you develop a plan that reflects what you want to achieve and identifies the steps to get there. They provide ongoing monitoring, help problem-solve when challenges arise, and celebrate your progress.

Targeted Case Management is available to adults with serious mental illness in our service area. Our case managers are experienced in working with individuals who may have had difficulty engaging with services in the past.

What to Expect

Your case manager will meet with you one to four times per month at our office, in your home, or in community settings. Between meetings, they coordinate services, make referrals, and advocate with other agencies on your behalf.

The goal is to help you build the skills and connections needed to manage your care more independently over time. Your service plan is adjusted as your needs and priorities change.

Therapeutic Rehabilitation

Our structured day program helps adults with serious mental illness build daily living skills, social connections, and confidence. TRP combines skill-building groups, therapeutic activities, and a supportive community setting.

Recovery Care Coordination

For those who have achieved some stability, Recovery Care Coordination provides consistent support to maintain your progress. Your coordinator helps you stay connected to resources and navigate life transitions.

Outpatient Mental Health

Receive professional therapy and counseling while maintaining your daily life. We offer individual, group, and family therapy using evidence-based approaches like CBT and DBT, with telehealth options available.

Outpatient Physician’s Clinic

Our outpatient psychiatric clinic provides comprehensive evaluations and medication management delivered by experienced psychiatrists and psychiatric nurse practitioners for a wide range of mental health conditions.

Intensive In Home Therapy

IIH brings comprehensive therapy directly to families in their home. Designed for children and adolescents who need more than traditional outpatient care, our therapists strengthen the entire family system with multiple sessions per week.
