Supports for Community Living Case Management

The Supports for Community Living (SCL) Waiver helps individuals with intellectual and developmental disabilities live in their communities with the residential and support services they need. RiverValley’s SCL case managers coordinate services and advocate for clients to ensure they receive the best possible care.

The SCL Waiver is a Kentucky Medicaid program designed to serve individuals with intellectual and developmental disabilities who meet the criteria for placement in an Intermediate Care Facility (ICF) but choose instead to receive services and supports within the community. The waiver provides funding for community-based residential care, including staffed residences and adult foster care homes, as well as a range of additional support services.

RiverValley offers SCL case management services for individuals diagnosed with a developmental disability prior to the age of 22 or for individuals under the age of 18 with intellectual disabilities and an IQ of 70 or below. Our case managers assist clients in developing natural supports and connecting with services that promote independent living and meaningful participation in their communities.

SCL Waiver services can include community living supports, personal care, adult day training, occupational and physical therapy, behavioral support, respite care, supported employment, and other services tailored to each individual’s needs and goals. Case managers work with participants, families, and service providers to ensure all supports are coordinated and delivered effectively.

To qualify, individuals must have a documented intellectual or developmental disability, meet Medicaid financial eligibility requirements, and require a level of care equivalent to that provided in an ICF. Psychological testing and adaptive behavior assessment are needed to verify disability. Please note that there is a waitlist for SCL Waiver allocation, managed by the Kentucky Department for Behavioral Health, Developmental and Intellectual Disabilities.

It is possible to receive services on one waiver while waiting for allocation on another, but you cannot access two waivers simultaneously. What to Expect

Getting started begins with a conversation. Our team will review your situation, explain the SCL Waiver eligibility requirements, and help you understand what documentation is needed. If you’re already on the SCL Waiver, we can begin case management services immediately.

Once enrolled, your case manager will work with you to develop an individualized service plan, coordinate with providers, and check in regularly to ensure your services are meeting your needs. We’re here to advocate for you and make sure you have access to everything the waiver provides.
